On balance, LMSCL Lower Mainland Society for Community Living demonstrated substantial conformance to the standards. LMSCL has since 2009 provided programs and services to extremely high-risk, vulnerable, and medically fragile adults, children, and youth living in Vancouver. The organization is recognized by funding and referral sources for its willingness to always accept very difficult placements and make a difference in the lives of persons served. Programs and services are highly valued by the persons served, their families, and other stakeholders. Among the many strengths of the organization are its dedicated workforce, accessible leadership, and commitment to ensuring a healthy and safe environment for all. Opportunities for improvement noted in this report include implementing procedures for addressing safety when services are provided at sites not owned/leased or controlled/operated by LMSCL, expanding procedures for performance appraisals, and ensuring that the analysis of service delivery and business function performance includes comparative analysis and identification of trends and causes. Opportunities for improvement are also noted in the areas of program/service structure and individualized plans. LMSCL appears to have the willingness and capacity to address the recommendations. LMSCL Lower Mainland Society for Community Living appears likely to maintain and/or improve its current method of operation and demonstrates a commitment to ongoing quality improvement. LMSCL Lower Mainland Society for Community Living is required to submit a post-survey Quality Improvement Plan (QIP) to CARF that addresses all recommendations identified in this report. LMSCL Lower Mainland Society for Community Living has earned a Three-Year Accreditation. The leadership team and staff are complimented and congratulated for this achievement.
