After-Hours Coordinator - Full Time

   

The After-Hours Coordinator is responsible for overseeing operations of residential programs,  Monday to Friday from 8pm to 4 am.

Education & Experience

  

  • University or college degree in healthcare, health management, or social services (Bachelor's or Masters)
  • Acceptable Criminal Record Search reports
  • Fluent in English, both written and verbal communication
  • First Aid/CPR Level C or higher, and NCI (or equivalent) Certificate
  • Valid class 5 driver’s license with no restrictions and a clear driving abstract
  • We are not accepting students for this position

Roles and Responsibilities

Client

  

  • Assists in planning supervised family visitations during weekends.
  • Transports clients to medical, dental, or other necessary appointments as needed.

Management and Staff

  

  • Establishes working relationships with the Management Team, Case Managers and Support Workers to facilitate after-hours evening and weekend support to all residential programs.
  • Receive updates from Case Managers before the start of a shift on the status of the program and clients and any specific tasks to follow or coordinate during the shift.
  • Supervises Support Workers on managing a crisis or emergency including facilitating hospitalization and police involvement.
  • Prepare a report to update Case Managers at the end of the shift on any concerns or issues requiring follow up.
  • Reviews and edits Critical Incident reports that happened in the shift.
  • Provides after hours response and support, either by phone or in person assistance if needed.
  • Provides staff debriefing of stressful and/or crisis situations.
  • Performs random phone checks of programs during the night and attends in persons if there are concerns.
  • Complies with other assignments and directions by the Case Management Supervisor and Services Manager. 

Administration

  • Assist scheduler and Services Manager with staff scheduling and shift schedules.
  • Assist in services Department general orientation of new employees
  • Oversee data tracking and collection for program outcomes. Analyze results with Quality Assurance Manager for program improvements.
  • Ensure all reports, updates, statistics, time sheets and petty cash records are reviewed and completed within deadlines.
  • Submit required weekly /monthly/ quarterly reports within deadlines.
  • Provide on-site support and training to Support Workers in areas of client data tracking, client file documentation and archiving, petty cash, reports, policy/procedures, technology, standards of care, house safety and maintenance.
  • Approve work hours and ensure accuracy of staff timesheets in the payroll system.
  • Comply with the assignments and instructions of Case Management Supervisor and Services Manager as part of the duties

Details of the Available Position- Medication Assistant

   

The Medication assistant assists in activities and tasks related to medication management in all programs of the company.

Education & Experience

    

  • University or college diploma as Pharmacy Assistant or similar topics - Canadian graduates are preferred.
  • Working knowledge and at least 1 year of experience in pharmacy or health care setting in Canada, dealing with medications and prescriptions.

Requirements

  • Physically fit and in excellent health.
  • Valid Class 5 driver's license with no restrictions and a good driving abstract is required.
  • No criminal convictions, no adult and criminal charges pending.
  • No history of improper conduct in all areas.
  • Must be fluent in both verbal and written English.

  • Knowledge of pharmaceutical terminologies and medication administration.
  • Ability to work effectively as part of a healthcare team.

Roles and Responsibilities

  

1- Monitoring and review

  • Reviews client binder and assists in maintaining accurate and up to date medication records for each patient.
  • Ensures medication is stored properly, administered correctly, and documented completely by the staff.
  • Checking medication if it is properly disposed in each program in a regular basis. Reports on any discrepancies in dispensing or administering medications.
  • Monitors implementation of dosage or frequency changes, and reports any incidents or discrepancies.
  • Helps in conducting regular audits of medication records to ensure compliance with the guidelines.

2. Medication Dispensing Support

  • Receives, reviews, and processes medication orders from healthcare providers.
  • Assist in preparing and dispensing medications to programs accurately.
  • Ensure proper labeling and packaging of medication.
  • Maintain a secure and organized medication storage area.

3. Inventory Management

  • Communicates with Case Managers and staff in each program to ensure medication and medical supplies are enough.
  • Receives and checks incoming medication supplies.
  • Collaborate with the Medication Coordinator to place orders for medications and supplies.

4. Administrative Tasks

  • Performs administrative duties such as filling. record-keeping, and data entry.

5. Training

  • Assists in providing training to staff regarding medication management

6. Medication Safety

  • Adhere to medication protocols to prevent medication errors.
  • Report any potential medication safety issues to the Medication Coordinator.
  • Assists in Coordinating with pharmacist regarding individuals’ medications.
  • Ensures home adhere to safety and other necessary medication management standards.
  • Fulfills other tasks relevant to the job description that is not listed above but assigned by the Medication and Nursing Coordinator.
